Costruita sotto il palazzo del Comune delle Serre, a pianta rettangolare aperta su tre lati attraverso ampie arcate. Vi aveva sede la compagnia di Sant’Elisabetta, poi unita nei primi anni del Seicento a quella di Santa Caterina, oggi Misericordia. Durante i lavori di restauro del 1922 riemersero gli affreschi quattrocenteschi. La lunetta centrale ospita la «Madonna col Bambino e i Santi Fabiano e Sebastiano a sinistra, Bernardino e Macario a destra», protettori della Comunità delle Serre. Sul lato destro è visibile un altro affresco raffigurante la «Madonna col Bambino», da riferire ad Andrea di Nicolò e sulla volta stellata, al centro, è dipinto «Dio padre benedicente».

Built under the Palazzo del Comune delle Serre, with a rectangular planimetry open on three sides through wide arches. Firstly the company of Sant’Elisabetta was based there, later joined to that of Santa Caterina alla Misericordia in the early seventeenth century. During the restoration work in 1922, the fifteenth-century frescoes re-emerged. In the central lunette is situated the «Madonna and Child with Saints Fabiano and Sebastiano on the left, Bernardino and Macario on the right» which are the protectors of the Community of the Serre. On the right side there is another fresco depicting “Madonna and Child”, to be referred to Andrea di Nicolò and on the starry vault, in the center, is painted “God the blessing father”.
